Please note that once your online registration is received, you must provide the following to complete the registration process:

1. Child’s birth certificate
2. Child’s immunization record
3. Proof of residence (utility bill, mortgage, or lease)

This verification process can be completed at your 4K site, the District Office, or any HSSD elementary school office.
